Chippewa County Healthy Communities Pass-through Fund

"Collaboratively improving community health and quality of life."

Chippewa County Healthy Communities Pass-through Fund is established to accept donation and process disbursements for Chippewa County Healthy Communities (CCHC), a collaborative community effort to enhance community health and the quality of life for residents of Chippewa County through educational and Prevention initiatives.

Chippewa County Healthy Communities (CCHC) began in 1994 when local residents and organizations came together to better understand and improve community health. What started as the Chippewa Health Improvement Partnership has grown into a countywide coalition of more than 90 members working together to create lasting, positive change. Today, CCHC continues to unite partners across sectors with one shared vision - a healthy and thriving community for all.

(CCHC) is a collaborative coalition that has worked with the community for more than 30 years to build a healthier and more connected community. CCHC brings people together to improve health and well-being for everyone in our county. We focus on community-identified priorities like access to care, mental health, and substance use prevention.

Vision: A healthy and thriving Community for all.

Mission: Strengthen the health and well-being of our community by working together and providing fair access to resources.

Goals: Empower residents to navigate healthcare and access basic needs, increase awareness of mental health and improve access to services, and enhance education and prevention related to alcohol and substance use.

Chippewa County Healthy Communities Pass-through Fund was established to enhance community health and the quality of life for residents.
